I've heard of folks having "the book thrown at them" in court, but you don't often see the judge throwin' a punch.

Ummm... how do officers of the court get into that kind of situation and no one gets arrested or fired on the spot?

The Judge is god. If the judge does not file a complaint, nothing happens. The attorney would (likely) never DARE file a complaint.
However, I do think it is possible for that attorney to file motions for any other cases he has to be removed from that judge's courtroom.
